What does a book map physically look like? Is it on a computer or real paper? Is there a website you recommend that shows you what a book map looks like?

I have used spreadsheets (not consistently though). Do most authors track “off camera” action too. That is, what secondary characters/villains are doing/where they are when not with the MC?
